The application form and ICR summary sheet are available from the Admission Cell of Dar-us-Salaam Educational Trust, located at Dar-us-Salam, Nampally, Hyderabad-500001. They can be obtained upon submission of a Demand Draft for Rs. 1500/- (Rupees fifteen hundred only) for general and backward class candidates, and Rs. 1000/- (Rupees one thousand only) for SC, ST, and P.W.D. (persons with disabilities) candidates. The Demand Draft should be made in favor of "Dept of Hospital Management (DSM)," payable at Hyderabad, and can be submitted either by post or in person.

Alternatively, the application form and ICR summary sheet can be downloaded from the websites www.dshm.co.in or www.osmania.ac.in. The completed application form and ICR summary sheet should be sent with the prescribed Demand Draft to the Admission Cell at, Dar-us-Salaam Educational Trust, Dar-us-Salam, Nampally, Hyderabad -500001.
